Here at Tasmanian Botanics we take all product complaints and patient experiences seriously.

Reviewing product complaints is a detailed and thorough process so we ask that you please fill the form out with the required details as completely and correctly as possible.

 

We require that the complaint form be completed for each product complaint with all the details on the form filled out where possible. Please do not include multiple products in one complaint form – this will prompt a request to re-submit the complaint properly.

Please provide a description of how you have stored the product since receiving it.

We will require photos of your product with original packaging. If you do not provide photos your complaint/feedback will be unable to be processed by our team. Please do not dispose of your product if there is a suspected defect, as we may request for the product to be returned for further investigation
